    Our Medical Records Department is here to support you with a wide range of documentation needs. We understand that managing your child’s health often involves coordination with schools, employers, community programs, and other organizations, and we are committed to helping make that process as smooth as possible.


    We can assist with completing and processing many types of forms and documentation, including:


    School and Education Forms

    • School and physical education (PE) forms
    • School entry forms (including lead screening, hemoglobin, immunizations, and TB documentation when required)
    • School nurse medication administration forms
    • College and university health forms
    • Camp forms
    • Work permits

    Medication Forms

    • Asthma medications (e.g., albuterol inhalers/nebulizers)
    • Epinephrine auto-injector (EpiPen) forms

    • DTA (Department of Transitional Assistance) forms

    Medical Records Services

    Our goal is to ensure that families have timely access to the documentation they need for school, work, and community support services. We encourage families to submit form requests in advance whenever possible, as some documents may require provider review and processing time.

    Referrals/Specialty Care
  • Nursing/Shot Visits

    We offer convenient vaccine (shot) visits in our office to help keep your child’s immunizations up to date. These visits are typically quick appointments where your child will be seen by one of our nurses rather than their Primary Care Provider (PCP).


    Most vaccine visits are scheduled on Tuesdays and Thursdays; however, we understand that every family’s schedule is different. If these days do not work for you, please contact our office and we will do our best to accommodate your needs.


    Routine vaccines are usually given during your child’s scheduled well-child physical exam. Nurse vaccine visits provide an additional option for:


    • Catching up on vaccines if your child is behind on immunizations
    • Receiving booster doses that are needed after an initial vaccine series
    • Completing school, camp, or activity vaccine requirements
    • Receiving vaccines that are due between physical exams

    Keeping your child’s vaccines up to date is an important part of protecting their health and the health of our community. Our nursing team is here to make the process simple and convenient for your family.

  • Refilling Your Medications

    We kindly request that you give our office a call Monday through Friday for any prescription refill needs you may have.  We are not always able to fill prescriptions on the weekends or Holidays so please plan accordingly.  


    Routine meds like albuterol, eczema creams, and allergy medications are usually filled without delay.  


    If you are calling for a refill of a controlled substance, like ADHD medications: Concerta, Adderall, Focalin please call when you still have 3-4 pills left to allow for ample time to locate supply.  


    Our team is dedicated to providing outstanding patient care and ensuring that your medications are readily available when you need them.

  • ADHD/ADD Evaluations

    Our trained professional staff will complete the initial evaluation if you have concerns that your child may be experiencing difficulties with focus, attention, or impulsivity. Before your child’s first appointment, you and your child’s teachers may be asked to complete Vanderbilt Assessment Forms to help us better understand your child’s symptoms and how they are affecting daily life at home and school.


    If your child is diagnosed with ADHD, our team will continue to provide ongoing care and support. Medication management requires regular follow-up visits with your child’s primary care provider. Your child will need to be seen in the office approximately every 3 months for medication rechecks, where we will monitor growth (height and weight), blood pressure, symptoms, and overall response to treatment.


    ADHD medications often require adjustments over time. Doses are typically started low and gradually adjusted as needed to find the medication and dose that best supports your child’s individual needs while minimizing side effects. Our goal is to work together with your family to create the most effective and personalized treatment plan for your child.

  • Newborn Care

    One of our providers, even if it is not your chosen Primary Care Provider, will check on your newborn at Charton Memorial Hospital after you give birth.  Prior to discharge you will  be directed to set up an initial newborn visit at the office.  We keep a close eye on your baby at this age and you may be in the office often during the first few weeks of life.

  • Chronic Care Management

    If your child has a disability, complex medical condition, or chronic illness, your healthcare journey may feel like the road less traveled. You may find yourself working with multiple specialists, therapists, schools, and community resources while trying to coordinate the many aspects of your child’s care. Our practice understands the unique challenges families face and is committed to providing guidance, support, and compassionate care throughout every stage of your child’s development.


    Children with chronic illnesses often require more than routine pediatric care. Our team partners with families to provide ongoing monitoring, education, care coordination, and support tailored to your child’s individual needs. We work to understand the whole child—not just the diagnosis—including their physical health, emotional well-being, development, school experience, and quality of life.


    We recognize that children with complex medical needs may require additional time and attention during visits. Some appointments may be longer than a typical well-child visit so that we can fully address your questions, review medical information, coordinate care, and make sure your concerns are heard. Our goal is to provide ample time to discuss your child’s health, medications, treatments, specialists, and any challenges your family may be experiencing.

    Chronic Care Management
  • Ear Piercing (Not provided in the office)

    Our office does not provide ear piercing services. Ear piercing is an elective procedure, and we recommend waiting until your child is at least 3 months old before having their ears pierced. Waiting until this age allows your baby to receive their first set of routine vaccines and gives the earlobe additional time to grow, which can help with more accurate placement of the earrings.


    Our goal is to help families make choices that keep procedures as safe as possible for their children. When choosing where to have your child’s ears pierced, we recommend selecting a professional location that follows safe piercing practices, including:


    1. Uses sterile, single-use equipment or properly sterilized instruments
    2. Uses hypoallergenic earrings (such as surgical-grade stainless steel, titanium, or 14k gold) to reduce the risk of irritation or allergic reactions
    3. Has experience piercing infants and young children
    4. Provides clear aftercare instructions
    5. Practices good hand hygiene and infection prevention measures
    6. Avoids piercing through areas other than the earlobe (cartilage piercing is not recommended for young children due to increased risk of infection)

    Families may consider professional piercing studios that specialize in children’s ear piercing or reputable jewelry stores with trained piercing professionals. Many families choose services such as:


    • Claire's — provides ear piercing services at many retail locations with trained staff
    • Local professional piercing studios that use sterile techniques and have experience working with young children

    After piercing, it is important to follow all aftercare instructions, keep hands clean when touching the earrings, avoid removing earrings too soon, and watch for signs of infection such as increasing redness, swelling, warmth, drainage, or worsening pain.
